About

QVAD Review Board IMS is the online platform for completing and submitting forms to the Voluntary Assisted Dying Review Board as required by the Voluntary Assisted Dying Act 2021. 

It is also where practitioners can submit an application to become an authorised voluntary assisted dying practitioner. 

QVAD Review Board IMS is not integrated with other clinical systems.

Digital ID
QVAD Review Board IMS uses Digital ID to confirm your digital identity.

To access QVAD Review Board IMS you must have registered for a with a standard level of identity strength.

You do not need access to QVAD Review Board IMS to submit the
following forms. However, if you do have access please log in first.

I have received a referral for consulting assessment.

I have received and disposed of a voluntary assisted dying substance .

I have completed a cause of death certificate for a relevant patient.
